Once your information is verified, I will email you your login information.
Once approved and if you are in PA, I will need a
PA Rev 1220 filled out and signed.

The above link will take you to the PA Dept Revenue's Main Site.
On the bottom left, you will see a search button.
Type 1220 in it, and it will take you to the Pa Rev-1220 Fill in Form.
Fill out the form above and I will send you a user-name and password to log into my wholesale account!
